Claude Code PreToolUse hook decision "approve"/"block" is deprecated — use hookSpecificOutput.permissionDecision

Fix
{
  "hookSpecificOutput": {
    "hookEventName": "PreToolUse",
    "permissionDecision": "deny",
    "permissionDecisionReason": "rm -rf outside the repo is blocked"
  }
}

Values: allow, deny, ask. Exit 0 and print the JSON to stdout (exit code 2 still blocks, with stderr fed back to Claude). permissions.deny rules win over a hook's allow.

Symptom

A PreToolUse hook prints {"decision": "approve", "reason": "..."} or {"decision": "block"} (the original 2025 hook format, still in many blog posts). It is handled through a compatibility mapping and can't express ask, updatedInput or extra context.

Cause

For PreToolUse, the top-level decision/reason fields are deprecated; "approve" maps to "allow" and "block" to "deny". Permission decisions moved under hookSpecificOutput (exposed to hooks in v1.0.59, 2025-07-23). Other events such as PostToolUse and Stop still use top-level decision/reason.

Verify

Run claude --debug and confirm the hook's decision is applied; a deny shows the reason to Claude.
