    "body": "## Symptom\nA PreToolUse hook prints `{\"decision\": \"approve\", \"reason\": \"...\"}` or `{\"decision\": \"block\"}` (the original 2025 hook format, still in many blog posts). It is handled through a compatibility mapping and can't express `ask`, `updatedInput` or extra context.\n\n## Cause\nFor PreToolUse, the top-level `decision`/`reason` fields are deprecated; `\"approve\"` maps to `\"allow\"` and `\"block\"` to `\"deny\"`. Permission decisions moved under `hookSpecificOutput` (exposed to hooks in v1.0.59, 2025-07-23). Other events such as PostToolUse and Stop still use top-level `decision`/`reason`.\n\n## Fix\n```json\n{\n  \"hookSpecificOutput\": {\n    \"hookEventName\": \"PreToolUse\",\n    \"permissionDecision\": \"deny\",\n    \"permissionDecisionReason\": \"rm -rf outside the repo is blocked\"\n  }\n}\n```\nValues: `allow`, `deny`, `ask`.
