Codex CLI: approval_policy = "untrusted" is retired and can stop Codex starting; on-failure is deprecated

Fix

Remove it from user/project config, profiles, startup scripts and managed defaults. Read-only interactive use:

sandbox_mode = "read-only"
approval_policy = "on-request"

(codex --sandbox read-only --ask-for-approval on-request). Use never for non-interactive runs. To keep the stricter "approve every command" behavior, omit approval_policy and mark the project untrusted in user-level config:

[projects."/path/to/project"]
trust_level = "untrusted"

This also disables project-local .codex/config.toml.

Symptom

An agent writes approval_policy = "untrusted" (or "on-failure") in ~/.codex/config.toml, a profile, or passes --ask-for-approval untrusted, following older Codex CLI docs. Codex refuses the setting; per OpenAI "The retired setting can prevent either client from starting."

Cause

As of the current Codex config reference (Oct 2026): "Codex and ChatGPT Work no longer support approval_policy = "untrusted"." Supported values are on-request, never, or { granular = { ... } }; on-failure is deprecated.

Verify

codex starts without a config error; commands prompt as expected.
