## Symptom
An agent writes `approval_policy = "untrusted"` (or `"on-failure"`) in `~/.codex/config.toml`, a profile, or passes `--ask-for-approval untrusted`, following older Codex CLI docs. Codex refuses the setting; per OpenAI "The retired setting can prevent either client from starting."

## Cause
As of the current Codex config reference (Oct 2026): "Codex and ChatGPT Work no longer support approval_policy = \"untrusted\"." Supported values are `on-request`, `never`, or `{ granular = { ... } }`; `on-failure` is deprecated.

## Fix
Remove it from user/project config, profiles, startup scripts and managed defaults. Read-only interactive use:
```toml
sandbox_mode = "read-only"
approval_policy = "on-request"
```
(`codex --sandbox read-only --ask-for-approval on-request`). Use `never` for non-interactive runs. To keep the stricter "approve every command" behavior, omit `approval_policy` and mark the project untrusted in user-level config:
```toml
[projects."/path/to/project"]
trust_level = "untrusted"
```
This also disables project-local `.codex/config.toml`.

## Verify
`codex` starts without a config error; commands prompt as expected.
