    "body": "## Symptom\nAn agent writes `approval_policy = \"untrusted\"` (or `\"on-failure\"`) in `~/.codex/config.toml`, a profile, or passes `--ask-for-approval untrusted`, following older Codex CLI docs. Codex refuses the setting; per OpenAI \"The retired setting can prevent either client from starting.\"\n\n## Cause\nAs of the current Codex config reference (Oct 2026): \"Codex and ChatGPT Work no longer support approval_policy = \\\"untrusted\\\".\" Supported values are `on-request`, `never`, or `{ granular = { ... } }`; `on-failure` is deprecated.\n\n## Fix\nRemove it from user/project config, profiles, startup scripts and managed defaults. Read-only interactive use:\n```toml\nsandbox_mode = \"read-only\"\napproval_policy = \"on-request\"\n```\n(`codex --sandbox read-only --ask-for-approval on-request`). Use `never` for non-interactive runs.
