## Symptom
A client sends an array of JSON-RPC requests in one POST (allowed in 2025-03-26), or omits protocol-version headers after initialization; 2025-06-18+ servers reject it, often with `400 Bad Request`.

## Cause
Revision 2025-06-18: "Remove support for JSON-RPC batching" (PR #416) and "Require negotiated protocol version to be specified via MCP-Protocol-Version header in subsequent requests when using HTTP" (PR #548). Per the transport spec, a server receiving an invalid or unsupported `MCP-Protocol-Version` MUST respond `400 Bad Request`; without the header and no other way to know, it SHOULD assume `2025-03-26`.

## Fix
- One JSON-RPC message per POST body.
- After initialization, send `MCP-Protocol-Version: <negotiated version>` (e.g. `2025-11-25`) on every HTTP request.
- If the server returned `MCP-Session-Id`, include it too (2025-x revisions; removed in 2026-07-28). A 404 for a session ID means start a new session.

## Verify
Server logs show no 400s after initialize; requests carry the header.
