## Error

```text
EACCES: permission denied, posix_spawn
```

## Symptom
A headless Claude Code job had a custom stdio MCP server configured as:
```json
{"mcpServers": {"pc": {"type": "stdio", "command": "/srv/app/host/pc_mcp.py", "args": []}}}
```
The server's tools never appeared. The model looked for them with ToolSearch several times, then reported that the server failed to connect with `EACCES: permission denied, posix_spawn 'stdio'`. The rest of the session ran normally, minus those tools, so this is easy to miss in headless runs.

## Cause
`command` pointed straight at a Python script that had no execute bit. It was written on another machine and synced without mode changes. Claude Code spawns `command` directly, with no shell, so the kernel refused with EACCES.

## Fix
Either make it executable (the script needs a valid shebang such as `#!/usr/bin/env python3`):
```bash
chmod +x /srv/app/host/pc_mcp.py
```
or launch it through the interpreter so the mode doesn't matter:
```json
{"mcpServers": {"pc": {"type": "stdio", "command": "/usr/bin/python3", "args": ["/srv/app/host/pc_mcp.py"]}}}
```
Make the deploy step preserve modes (`rsync -a`) or set them explicitly.

## Verify
After `chmod +x` and a redeploy, the same test job connected to the server and used `mcp__pc__Edit` / `mcp__pc__Write` successfully ("the pc MCP server connected fine this time (no EACCES)").

## Notes
In headless runs, check `system/init` in the stream-json output: its `tools` and `mcp_servers` fields show whether the server connected. Don't rely on the model to notice. The server config schema also accepts `"alwaysLoad": true`, which was used so the tools sit in the initial tool list instead of behind ToolSearch. Its effect wasn't measured separately.
