## Symptom
An agent builds a remote MCP server the 2024 way: a GET `/sse` endpoint that emits an `endpoint` event plus a separate POST `/messages` endpoint (e.g. `SSEServerTransport`). Newer clients try to POST `initialize` to the URL and get 400/404/405.

## Cause
The HTTP+SSE transport from protocol 2024-11-05 was replaced by Streamable HTTP in 2025-03-26. Revision 2026-07-28 formally lists HTTP+SSE as Deprecated (SEP-2596), earliest removal three months after SEP-2596 reaches Final. Standard transports are now stdio and Streamable HTTP only.

## Fix
Serve one MCP endpoint (e.g. `https://example.com/mcp`):
- Client sends every JSON-RPC message as an HTTP POST with `Accept: application/json, text/event-stream`.
- For a request, server answers with either `Content-Type: application/json` (one object) or `text/event-stream`. Notifications/responses get `202 Accepted`.
- Validate `Origin`; respond 403 when present and invalid; bind to 127.0.0.1 locally.
In the official SDKs use the Streamable HTTP server transport instead of the SSE one. To keep old clients working, host the old SSE + POST endpoints alongside the new endpoint. Clients falling back: POST `InitializeRequest`; on 400/404/405, GET the URL and expect an `endpoint` SSE event (old transport).

## Verify
`curl -X POST https://host/mcp -H 'Accept: application/json, text/event-stream' -H 'Content-Type: application/json' -d '{"jsonrpc":"2.0","id":1,"method":"server/discover"}'` (2026-07-28) or an `initialize` request (2025-11-25 and earlier) returns a JSON-RPC result.
