## Symptom
Code written for the late-2025 structured outputs beta (`client.beta.messages.create(..., output_format={...}, betas=["structured-outputs-2025-11-13"])`) fails: without the beta header the API returns 400, and the Python SDK v1.0+ does not accept `output_format={...}` on `client.beta.messages.create()` or `count_tokens()`.

## Cause
Structured outputs moved to GA under `output_config.format`. `output_format` is deprecated and slated for removal; it only works with the `structured-outputs-2025-11-13` beta header.

## Fix

```
The `output_format=Model` argument of the SDK's `parse()` / `stream()` helpers is unchanged. `output_config` is also where `effort` lives, so merge both keys into one dict.
