    "title": "Claude Code MCP stdio server \"EACCES: permission denied, posix_spawn\": the script used as command is not executable",
    "error": "EACCES: permission denied, posix_spawn",
    "body": "## Symptom\nA headless Claude Code job had a custom stdio MCP server configured as:\n```json\n{\"mcpServers\": {\"pc\": {\"type\": \"stdio\", \"command\": \"/srv/app/host/pc_mcp.py\", \"args\": []}}}\n```\nThe server's tools never appeared. The model looked for them with ToolSearch several times, then reported that the server failed to connect with `EACCES: permission denied, posix_spawn 'stdio'`. The rest of the session ran normally, minus those tools, so this is easy to miss in headless runs.\n\n## Cause\n`command` pointed straight at a Python script that had no execute bit. It was written on another machine and synced without mode changes. Claude Code spawns `command` directly, with no shell, so the kernel refused with EACCES.\n\n## Fix\nEither make it executable (the script needs a valid shebang such as `#!/usr/bin/env python3`):\n```bash\nchmod +x /srv/app/host/pc_mcp.py\n```\nor launch it through the interpreter so the mode doesn't matter:\n```json\n{\"mcpServers\": {\"pc\": {\"type\": \"stdio\", \"command\": \"/usr/bin/python3\", \"args\": [\"/srv/app/host/pc_mcp.py\"]}}}\n```\nMake the deploy step preserve modes (`rsync -a`) or set them explicitly.\n\n## Verify\nAfter `chmod +x` and a redeploy, the same test job connected to the server and used `mcp__pc__Edit` / `mcp__pc__Write` successfully (\"the pc MCP server connected fine this time (no EACCES)\").\n\n## Notes\nIn headless runs, check `system/init` in the stream-json output: its `tools` and `mcp_servers` fields show whether the server connected. Don't rely on the model to notice. The server config schema also accepts `\"alwaysLoad\": true`, which was used so the tools sit in the initial tool list instead of behind ToolSearch. Its effect wasn't measured separately.",
