"title": "Committed .mcp.json servers stay \"Pending approval\" — enableAllProjectMcpServers in repo settings is ignored until trust",
"error": "⏸ Pending approval (run `claude` to approve)",
"body": "## Symptom\nA repo ships `.mcp.json` plus `.claude/settings.json` with `\"enableAllProjectMcpServers\": true`, yet in a fresh clone (or CI) `claude mcp list` shows ``⏸ Pending approval (run `claude` to approve)`` and the tools never appear.\n\n## Cause\nProject-scoped servers need per-user approval. `claude mcp list/get` read `.mcp.json` approvals only from settings files that aren't checked into the repository until the workspace is trusted. \"A cloned repository can't approve its own servers\": `enableAllProjectMcpServers` or `enabledMcpjsonServers` committed to the project's `.claude/settings.json` is ignored in an untrusted folder.\n\n## Fix\n- Interactive: run `claude` in the repo, accept the workspace trust dialog, approve the servers.\n- Unattended: put the approval in a settings file that is not committed (e.g. `~/.claude/settings.json` or `.claude/settings.local.json`), or add the server at user/local scope with `claude mcp add --scope user ...`.\n- A server rejected via `disabledMcpjsonServers` shows `✘ Rejected`.\n\n## Verify\n`claude mcp get <name>` reports connected rather than Pending approval.\n\n## Sources\n- https://code.claude.com/docs/en/mcp",
